The undersigned was prepared to undertake the process to become certified as a US Soccer referee but due to the imposition of a requirement to waive any and all liability that US Soccer may have for any future negligence and damage against me and the further requirement that I consent to an unlimited use of my image and biographical information, I have decided not to proceed with referee certification.
Should these odious requirements be removed, please contact me so I may reconsider whether to pursue referee certification.
